不牢固的头戴式组件也可能对慢性脑开放灌流微透析（cOFM）取样过程产生负面影响。对术后植入的cOFM导向套管外部组件进行操作，可能导致套管尖端在脑内发生轻微的角度偏移，从而对邻近尖端的微小血管造成虽轻微但显著的损伤。这可能导致血脑屏障（BBB）完整性受损，并使脑组织间液（ISF）可能被血液成分污染...

由于脑脊液（ISF）介导的复杂且多模式的细胞间信号传导，研究中枢神经系统（CNS）疾病极具挑战性1,2,3,4。这种复杂性在胶质母细胞瘤（GBM）等脑肿瘤中尤为显著，其中细胞间信号在肿瘤细胞和基质细胞之间传递5,6,7,8,9。在许多情况下，多种生物分子（包括代谢物和蛋白质类信使）的异常交换促进了中枢神经系统疾病的发病机制。这一研究难题在GBM中尤为突出，因为ISF的分子组成具有时间动态性，并随着疾病进展以及干预性治疗的实施而不断演变10,11。
传统的胶质母细胞瘤临床前研究方法包括 体外 研究和单终点动物模型。这些方法由于无法同时捕捉肿瘤微环境的特征以及时间上的纵向动态变化，限制了其更广泛的临床应用。20世纪80年代初期取得的进展通过脑内微透析（cMD）技术的发展克服了这些情境局限性。12,13,14,15自45年前问世以来，cMD已确立为活体脑组织间液（ISF）纵向采样的标准方法16通过高浓度脑间质液溶质在半透膜中的自由扩散和对流作用，进入持续流动的低溶质浓度灌注液（通常为人工脑脊液，aCSF），从而实现这一过程。17这些半透膜由&#160;多种生物相容性聚合物，其常见的分子量截留范围从20 kDa到3 MDa18该微创手术能够实现纵向观察 原位 采集多种多样的分析物，范围从小分子（如代谢物或神经递质）到大分子蛋白质（如抗体）。然而，cMD膜的结构孔隙性及其组成聚合物的吸附特性可能会显著影响具有特定生物化学或尺寸特征的分析物的相对回收率，例如脂质或抗体。19,20.
为克服cMD膜的这一局限性，自2010年代初起开始出现技术进展，其中脑部开放流动微透析（cOFM）技术得以发展21。cOFM与cMD遵循相似的基本原理，但通过采用一种具有低吸附性、宏观开口约为100 µm的塑料网格探针，取代了半透膜，从而消除了由半透膜带来的生化限制20。大量研究报告表明，与cMD相比，cOFM在收集可检测的脑间质液成分方面具有更优的实用性，这些成分包括肽类激素、纳米抗体/抗体、小分子脂溶性治疗药物以及聚乙二醇化脂质体19,22,23,24,25。先前有关cOFM导引探针植入后生理组织反应的研究显示，血脑屏障的完整性在植入后15天内得以恢复，且在植入后30天内未形成胶质瘢痕26。由于以宏观开口替代了微透析膜，研究人员还证明了cOFM导引探针具有创新应用潜力，可用于将异种移植的胶质瘤细胞注入免疫缺陷大鼠脑内，从而实现无创的肿瘤移植与生长，并直接围绕cOFM采样区域形成肿瘤组织27。

Use of Cerebral Open-Flow Microperfusion for the Longitudinal Collection of Interstitial Fluid in an Animal Model of Glioblastoma

根据基础病理学特征和实验动物模型的具体性质，可在实验的中间阶段或最终阶段进行多种评估性测量，以确定实验是否成功。针对胶质母细胞瘤（GBM）生物学的研究，生物发光成像（BLI）可作为中间评估指标，用于评估cOFM导引介导的肿瘤细胞移植效果（图4A）。然而，由于植入的头部固定装置的物理特性可能在不同动物间比较肿瘤大小时削弱检测到的发光强度，因此该方法仅适用于简单的二分法评估，即判断肿瘤是否存在。

我们正在鉴定脑肿瘤间质液中参与疾病进展和治疗反应的可溶性信号分子。评估脑肿瘤间质内溶质的绝对浓度仍是一项研究挑战，需要采用复杂的实验方法。首先，使用无菌手术刀，在颅骨表面沿中线做矢状切口，从两眼之间开始，向尾侧延伸8至10毫米。然后使用无菌镊子从正中切口向侧方牵拉皮肤。确保前囟和后囟均可见，必要时延长切口。接着，用无菌手术刀切开骨膜，并用镊子将组织向侧方牵拉。使用无菌棉签蘸取1%至3%的过氧化氢清洁颅骨表面。然后使用0.3至1.0毫升的胰岛素注射器将20至40微升的自酸蚀牙科粘合剂均匀地涂布在颅骨表面。接着用牙科固化灯照射，使粘合剂固化。固化后，用无菌棉签和生理盐水擦去多余的粘合剂。将钻头重新定位至cOFM导板坐标处。将钻速设置为每分钟15,000转，并使用DV显微操作仪旋钮钻出骨窗。在钻孔过程中需频繁监测颅骨厚度，并用生理盐水冲洗以清除碎屑并降低热量。在每个锚定螺钉坐标处重复钻孔操作。随后使用无菌镊子和无菌螺丝刀安装锚定螺钉，直至螺钉插入深度达一至两个螺纹长度。然后将cOFM导向装置和假体组件安装到导向支架中，并将支架固定至立体定位仪框架上。将导向尖端置于颅骨钻孔上方，以每分钟1毫米的速度缓慢下降至预定深度。在颅骨上均匀涂抹薄层粘结剂，覆盖cOFM导向装置下方及周围区域以及螺钉周围区域。在涂抹下一层之前，使用牙科固化灯使每一层材料充分固化。确保粘合剂覆盖各部件之间的整个区域，同时避免接触锁定楔块。将引导支架从cOFM引导装置上取下，并使用显微操作仪的DV旋钮缓慢抬起。使用镊子从cOFM导向器和假体组件中取出锁定楔块。轻轻将假体从导向器中取出。使用ML和AP微操作仪旋钮，将注射器灌注插入组件对准cOFM导向器上方。使用 DV 旋钮降低灌注插件，并用镊子牢固地重新插入锁定楔块以固定其位置。然后从注射器中以每分钟 1 微升的速度注入 3 微升的细胞悬液。观察 cOFM 导向器周围的区域，确保没有细胞悬液从组件中流出。使用镊子从引导装置和输注插件组件中取出锁定楔块。然后使用 DV 调节旋钮小心地将输注插件从引导装置中提起。使用气密玻璃注射器，用人工脑脊液预充满推注和抽吸管路。将管路的蠕动段安装到微灌注泵的泵头中。使用鲁尔锁接头将灌注液袋连接至推送管路，然后使用带法兰接头将采样插件连接至管路。将较长的进液管连接到推液管路的输出端，将较短的出液管连接到拉液管路的输入端。将连接好的采样插件放入含有750 µL人工脑脊液的无菌微量离心管中。使用带法兰的管路接头将组分收集器针头连接至拉液管的输出管路。然后以每分钟10 µL的速度冲洗导管20分钟，以去除气泡。接着，使用无泄漏注射器用人工脑脊液预填充拉管管线。将蠕动泵管重新安装到微灌注泵的泵头中。使用带法兰的管路连接件重新连接采样插件。将入口端连接至抽吸管路的输出端，出口端连接至抽吸管路的输入端。将插件放入含有750 µL人工脑脊液的无菌微量离心管中。以每分钟10 µL的流速再次进行20分钟的管路系统冲洗，以清除任何残留的气泡。将麻醉后的动物放入合适尺寸的背心式外套中，将前肢完全穿过袖孔，并沿动物背部牢固地固定好外套。使用镊子将导向器和假体组件中的锁紧楔块取出。然后小心地将假体插芯从导向器中提起。轻柔地将采样插芯安装到导向器中，并重新插入锁紧楔块。将动物从麻醉状态中取出，放入旋转笼系统中。将笼具的系绳连接到动物背心上，并启动旋转笼系统。cOFM采样结束后，关闭旋转笼系统，并将笼具系绳从动物背心上拆下。现在使用镊子从导引装置和采样插件组件中取出锁定楔块。小心地将采样插件从导引装置中提起，然后轻轻将假插件重新插入导引装置中，并重新插入锁定楔块。松开动物背心背面的固定件，并轻轻地将背心从动物身上取下。生物发光成像证实了经cOFM导管介导肿瘤细胞移植的动物体内存在肿瘤。对cOFM采集的间质液进行蛋白质组学主成分分析，结果显示替莫唑胺处理组与二甲基亚砜处理组动物之间存在明显分离。全局代谢组学信号分布在替莫唑胺组与二甲基亚砜组样本之间几乎完全重叠。尽管存在信号重叠，代谢组学火山图分析仍揭示了由替莫唑胺引起的显著变化。对同一动物代谢组学数值的定量比较显示，再循环构型在提高分析物浓度的同时，保持了代谢物的相对比例。脑肿瘤微环境中营养物质的相对可利用性仍缺乏深入了解。本方案旨在弥补这一空白。通过表征肿瘤微环境中的营养物质可利用性及其信号传导，可以阐明胶质瘤进展及对治疗产生适应性耐药的机制。
